Successful Surgical Removal of a Giant Anterior Mediastinal Thymolipoma
A 26-year-old man presented to Ethio Tebib Hospital with a one-year history of progressive shortness of breath on exertion, intermittent dry cough, easy fatigability, and nonspecific chest discomfort, with worsening symptoms over the preceding four months. He also reported episodes of breathlessness while lying flat. He had no history of smoking, pulmonary tuberculosis, chronic medical illness, or occupational exposure.
Comprehensive evaluation, including chest CT imaging and CT-guided core needle biopsy, revealed a giant anterior mediastinal fatty mass causing significant compression of the surrounding mediastinal structures. Following multidisciplinary assessment, the patient was scheduled for definitive surgical excision.
Our Cardiothoracic Surgery team successfully performed a median sternotomy with complete excision of the mediastinal mass.
Intraoperatively, the tumor was found to extend into both sides of the mediastinum, with the bulk occupying the left hemithorax. Careful dissection and ligation of the feeding vessels enabled complete removal of the mass while preserving the surrounding vital structures. A mediastinal drain was placed, and the procedure was completed successfully without intraoperative complications.
The patient had an uneventful postoperative recovery with marked improvement in his respiratory symptoms.
Follow-up chest radiography demonstrated expected postoperative findings, including minimal bilateral pleural effusion with the chest tube in situ.
Histopathological examination of the excised specimen confirmed thymolipoma with no evidence of malignancy.
